Q. Are Racial Modifiers included in skills as listed for a monster?

A. Yes.

If the monster has this skill block:

Skills: Perception +8;

Racial Modifiers: Perception +4

You can just use the +8 Perception. The "Racial Modifiers" is just telling you where some of the bonus is coming from.
